Afterschool care
I give consent for my child/children to participate in Waimarinos OSCAR After School Programme. I understand that on occasion there may only be one adult with one child in a vehicle at any one time. Should my child need any medical attention due to sickness or accident I give my permission for Waimarino to seek the required qualified medical attention on my behalf. If my child suffers from any medical condition or requires medication I will notify Waimarino immediately and complete a medical consent form. It is my responsibility to provide any relevant information about my child/ren's behavioural difficulties and needs before attending Waimarino, or as soon as there are any changes. I understand that Waimarino has the right to alter or change any of the activities in the programme. I understand the sunscreen policy that requires my child to wear a rash shirt or wetsuit during all water activities.
I understand that if my child does not attend one of our feeder schools, I am required to organize transport to the Waimarino after school programme for my child. I understand that there will be safety rules, boundaries and regulations that will need to be followed and understand that if my child is not adhering to these rules they may be suspended or expelled from the programme. I understand that Waimarino classifies bullying as ruining the enjoyment of others and that if my child is not adhering to these rules they may be suspended or expelled from the programme. If my child is suspended or expelled I understand that there will not be a refund. I also understand that Waimarino does not accept any responsibility for any lost, stolen or damaged items that my child may bring to the programme.
I understand that this enrolment is until the end of the term period booked. I understand that all after school programme invoices must be paid in full by the due date. There are no refunds if my child doesnt attend the Waimarino after school programme, nor do we offer catch up days. We require two weeks written notice to change or amend your enrolment. I understand that credit card fees are non-refundable. For any invoices not paid by their due date, I will be charged a late payment fee of 10%. Also, if at the time of a new invoice being generated there is still an outstanding account balance, there will be a further late payment fee of 10% charged.
Holiday Programme
I give consent for my child/children to participate in Waimarinos OSCAR Holiday Programme. I understand that on occasion there may only be one adult with one child in a vehicle at any one time. I understand that this enrolment is until the end of the holiday programme booked. By enrolling in this holiday programme I consent to my child being taken offsite on field trips. I have checked that the booking made is correct and that changing the session my child/ren are booked into after the booking is finalized can not be guaranteed. I understand that all holiday programme invoices must be paid in full by the due date unless a payment plan has been discussed prior to the due date. There are no refunds if my child doesnt attend the Waimarino holiday programme, nor do we offer catch up days. We require two weeks written notice to change or amend your enrolment. I understand that credit card fees are non-refundable. I understand that bus fares and meals are non-refundable in the event that my child is unable to attend due to illness or injury. For any invoices not paid by their due date, I will be charged a late payment fee of 10%. If you are applying for a WINZ subsidy please be aware that in the event that you are not eligible for the full subsidy, you will be required to pay. WINZ subsidies may not be able to cover meals or bus fares. I understand that if I am applying for a WINZ subsidy it is my responsibility to provide the required paperwork to the OSCAR Manager or to contact them for assistance.
Privacy Act 2020. I / We authorise Waimarino Education Trust to collect and retain information that regards the health and safety of your child (in case of emergency). The privacy principles are the core of the Privacy Act, and need to be adhered to when dealing with the following;
- The collection of personal information
- The storage of personal information
- The use of personal information
This information is also made available to the MSD for their audit purposes. I also understand that if any of the staff of Waimarino has any concern for the safety of my child or any of their family members, that they may inform the relevant authorities. I understand that my information collected via enrolment and profiles on Enrolmy can be shared with relevant authorities and or providers under the Sharing Information - Family Violence Act 2019 and the Oranga Tamariki Act.
Privacy Information Disclaimer
Waimarino reserves the right to exclude individuals from any of their OSCAR programmes if the information provided does not meet the minimum prescribed, accreditation standards to ensure that the duty of care for any child can be met. These standards are mandated and audited by the sectors governing body, the Ministry of Social Development.
